Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community. 

Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description

The opportunity:
We are looking for an Occupational Therapists to join our supportive and professional Adult Allied Health multidisciplinary team in a Part Time Fixed Term Contract capacity.

Responsibilities include but are not limited to;

  • Provision of quality Occupational Therapy services to individuals and their support networks; including assessment (initial and ongoing as required), intervention, individual and group-based programs, education and support services.
  • In collaboration with the Allied Health Team- identify, organize, plan, implement and evaluate evidence-based services.
  • Participation in continuous improvement and planning processes.
  • Liaison with key stakeholders and local service providers and participate in appropriate networks as required.
  • Maintenance of client records in accordance with professional and organisational protocols.
  • Participation in regular interdisciplinary team meetings, case reviews and case presentations
  • Completion of timely and accurate collection of appropriate data to meet relevant reporting requirements and to ensure compliance with privacy legislation

Desired Skills and Experience

Our ideal candidate:

  • Bachelor of Occupational Therapy or equivalent.
  • Registration with Australian Health Practitioners’ Regulation Authority (AHPRA)
  • Minimum green registration with the State-Wide Equipment Program (SWEP)
  • Background and interest in Dementia (including young onset Dementia) highly desirable
