Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description

The opportunity:

We are looking for a Social Worker (Neighbourhood Care Team), working across three clinical sites in Melton on a part-time (0.6 FTE), fixed-term 18‑month basis.

We’re excited to introduce a new initiative designed to reshape how health is experienced in Melton. This program puts people at the centre—partnering closely with those facing chronic illness and complex circumstances to create a supportive, compassionate, and empowering pathway to better health.

In this role, you’ll work alongside GPs, a Clinical Care Coordinator, and a passionate multidisciplinary team to support individuals living with chronic conditions and complex psychosocial needs. Your expertise will be central to providing thoughtful psychosocial assessments, impactful counselling, and practical, real-world support that helps patients navigate life’s challenges and improve their wellbeing.

You’ll play a vital role in connecting people to the right community and social care supports, shaping personalised care plans, and breaking down barriers related to mental health, housing, family violence risk, financial hardship, social isolation, and other social determinants of health. Desired Skills and Experience

About you

You are an AASW-registered Social Worker who thrives in collaborative environments and is passionate about delivering holistic, person-centred care. With experience in—or closely aligned to—primary care or general practice, you bring a strong foundation in psychosocial assessment, brief intervention, and counselling, and a genuine commitment to improving patient outcomes.

You excel at working within multidisciplinary teams, contributing to shared care planning and partnering effectively with GPs and other providers. Your strengths in advocacy and system navigation enable you to connect people with the supports they need, while your sound judgement ensures you can confidently manage risk, safeguarding, and mandatory reporting responsibilities.

A skilled communicator, you build trust with patients, carers, and colleagues through a trauma-informed and culturally safe approach. You take pride in maintaining high-quality clinical documentation and data integrity, and demonstrate a strong commitment to clinical governance, compliance, and continuous improvement.
