Job Description
The opportunity:
We are looking for a Clinical Care Coordinator (Neighbourhood Care Team), working across three clinical sites in Melton on a part-time (0.8 FTE), fixed-term 18‑month basis.
We’re excited to introduce a new initiative designed to reshape how health is experienced in Melton. This program puts people at the centre—partnering closely with those facing chronic illness and complex circumstances to create a supportive, compassionate, and empowering pathway to better health.
As the Clinical Care Coordinator, you’ll play a vital role in delivering compassionate, person-centred care within the Neighbourhood Care Team. You’ll be a trusted partner for patients navigating chronic conditions and complex life circumstances—ensuring they feel heard, supported, and empowered every step of the way.
Guided by a commitment to collaboration and holistic care, you’ll work alongside GPs and practice teams to bring multidisciplinary care to life. By leading case conferences and fostering shared decision-making, you’ll help create coordinated, thoughtful care that truly meets the needs of each individual.
You’ll take a proactive, strengths-based approach—listening deeply, conducting comprehensive assessments, and co-designing personalised care plans that reflect what matters most to each patient. Through strong relationships and seamless coordination, you’ll connect people to the services and supports that enable them to live well.
With integrity and accountability at the core of your practice, you’ll maintain high-quality clinical records, capture meaningful data (including PAM-10 and EQ-5D-5L), and contribute to continuous learning and improvement across the team.
Desired Skills and Experience
Our ideal candidate:
You are a passionate and experienced Registered Nurse or Allied Health professional with current AHPRA registration with experience working in general practice and primary care and is committed to delivering truly patient-centred outcomes.
You bring extensive experience supporting people with chronic conditions, with a strong ability to empower your patients through self-management and effective behaviour change approaches. Your natural strengths in advocacy and navigating access to available supports will enable you to guide individuals through complex health and social care systems with confidence and compassion.
