Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The opportunity:
We are seeking a Senior Program Manager – Complex and Integrated Care to join our supportive and professional multidisciplinary teams in an ongoing, full‑time capacity, working across Box Hill and Eltham.
Responsibilities include but are not limited to:
- Provide strategic and operational leadership, supervision and support to the complex and integrated care program to enable a positive and engaged workforce.
- Develop and oversee budgets and financial sustainability ensuring resource allocation matches the needs of the service, community, and budget.
- Collaborate with staff and key stakeholders to optimise processes, workflows and client outcomes.
- Contribute to the design and development of service models that meet funder, referrer, and consumer expectations, and oversee service delivery projects and their evaluation.
- Provide comprehensive financial and program oversight to enable Executive decision‑making on viability, efficiency, and future resource management options.
Our ideal candidate:
- A tertiary qualification in nursing or an allied health discipline.
- 3–5 years’ management experience leading high performing teams that deliver efficient and effective services.
- Demonstrated ability to positively lead a team through change.
- Strong engagement and interpersonal skills with demonstrated capacity to establish, manage and maintain internal and external stakeholder relationships.
- Demonstrated skills in business development, including the achievement of identified targets and budget management.
- Demonstrated experience working in health or community‑based environments.
- A current Victorian Driver’s Licence.
