Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.

Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description

The Opportunity

We are looking for a Service Facilitator – Healthy Ageing at Home.to join our supportive and professional multidisciplinary teams in a contracted full-time position.

Responsibilities include but are not limited to;

  • Client & Family Support and Care Coordination: Provide high-quality support to Healthy Ageing at Home clients and families, manage referrals and intake, explain HealthAbility services, prioritise urgent needs, and ensure timely, effective service delivery.
  • Staff Coordination and Operational Management: Coordinate and roster support staff (including buddy shifts), participate in task rotations, manage schedules and documentation in IT systems, and ensure consistent service coverage within program guidelines.
  • Compliance, Risk Management, and Professional Practice: Ensure privacy and confidentiality compliance, work within role boundaries, escalate risks or issues to management, and align all service delivery with HealthAbility values and national standards.

Desired Skills and Experience

Ideal Candidate

  • Customer Service, Communication & Stakeholder Engagement: Strong customer service background with high-level phone, interpersonal, and communication skills, including sensitive engagement with culturally and linguistically diverse clients, people with complex needs, and internal/external stakeholders.
  • Health/Aged Care Administration & Systems Competence: Recent experience in health, aged care or disability settings, with strong administrative, rostering, and client management system skills, accurate record-keeping, reporting capability, and proficiency in Microsoft Office.
  • Adaptability, Problem-Solving & Team CollaborationDemonstrated flexibility, problem-solving ability, effective time management, and capacity to work both independently and collaboratively within multidisciplinary teams while contributing positively to continuous improvement and workplace change.
