Job Description

The opportunity:
We are looking for a Clinical Nurse Lead to join our supportive and professional multidisciplinary team in a ongoing full time capacity.

Responsibilities include but are not limited to;

  • Lead and deliver the Integrated Team Care (ITC) program as the primary clinician, providing culturally safe, holistic, and coordinated outreach care to Aboriginal and Torres Strait Islander clients.
  • Manage a complex caseload of clients experiencing multiple chronic conditions (e.g. diabetes, cancer, heart, liver, renal, and respiratory disease), often compounded by psychosocial factors, health literacy challenges, and barriers to accessing mainstream services.
  • Provide both clinical leadership and direct nursing services within community health and Support at Home (SAH) programs.
  • Deliver person-centred, evidence-based nursing care including:
    • Outreach clinical nursing services for support at home clients. Duties include but are not limited to: medication administration, wound management, continence management, catheter and stoma bag changes, post hospital discharge follow up and clinical reviews
    • Provide community health nursing service including but not limited to participation as a key worker/care co-ordinator in the heart health, good life cancer survivorship and other chronic disease programs.

Desired Skills and Experience

Our ideal candidate:

  • Demonstrated leadership experience within community or primary care services (non-acute sector), including the ability to establish systems, drive service development, and lead clinical practice without pre-existing structures.
  • Proven ability to operate effectively in ambiguity and “grey areas”, exercising sound clinical judgment, initiative, and independent decision-making in complex and evolving situations.
  • Extensive experience (minimum 7 years) in community health nursing with a proven record of delivering complex, holistic care in home and community settings including intervention-based nursing e.g. experience in wound dressing, medication administration etc.
  • Advanced clinical competence and confidence in managing clients with multiple comorbidities, including chronic disease, mental health, and social complexity.
